We are looking for a Process Coordinator for TSB polyclinic

Then this could be the position for you! TSB Polyclinic is now looking for a Progress Coordinator. The position has been newly created and will have a particular focus on coordinating good patient outcomes and integrated aftercare. Who are we? We are an interdisciplinary professional group with a great commitment to our patients. The polyclinic's mission is to provide good, responsible health care and effective treatment to people with drug and addiction disorders. The target group is young people and adults aged 16 and over who have a problem with substance abuse and mental disorders, or other addictions, that require treatment. The outpatient clinic has an LAR team that works with patients who receive drug-assisted rehabilitation, and a TSB assessment team that assesses all referrals to both the outpatient clinic and inpatient treatment, and assists with information, guidance and coordinates assistance teams for municipalities in Telemark when necessary in difficult cases. We participate in research and development in the drug field. TSB Polyclinic covers the whole of Telemark, with polyclinic bases in Kragerø, Notodden and Skien. The position in question will mainly be linked to our professional environment in Skien. We are proud to be a workplace with high levels of well-being and a good working environment. We are now going to raise patient follow-up further to ensure that our patients experience seamless patient progression and holistic, integrated aftercare. Each patient must have a process coordinator who follows them throughout the entire course of treatment. If there is a need to switch to 24-hour treatment, the same therapist or coordinator must continue to follow the patient until all measures in the specialist health service have been completed. Close interaction with the municipality, NAV, GP and relatives is an important prerequisite for successful treatment and contributing to lasting results. The candidate will have the opportunity to help shape tasks and structure in the role, and must also be prepared to do clinical work such as having their own patient portfolio in treatment. Since the position is newly created, the job description and content will be evaluated continuously, and may be changed based on operations and needs.

• Coordinating patient processes in line with national patient processes and integrated aftercare
• treatment in line with national professional guidelines in TSB
• Being the patient's regular contact person and contributing to security, continuity and predictability throughout the entire treatment process.
• Ensuring progress in patient processes and preventing unnecessary delays.
• Planning and coordinating transitions between outpatient treatment, inpatient treatment and municipal services.
• Work closely with therapists, departments and external partners to ensure comprehensive services.
• Participate in treatment meetings and contribute to the follow-up of decisions and measures.
• Guide and inform patients and relatives about the course of treatment, rights and current offers.
• Contribute to the development and quality assurance of the patient course, and ensure good interaction routines with, among other things, private contracting parties.
• Identify and follow up on deviations or bottlenecks in the patient processes.

Qualifications

• Bachelor in social studies or Norwegian authorization as a nurse/care worker
• Experience from coordinating intake, patient processes, rights assessment, treatment and the like within the TSB field is an advantage
• further education in mental health and drug treatment is emphasized
• A command of the Norwegian language in writing and orally is a prerequisite for a sound professional practice
• Driver's license class B
